  (Birth Certificate (original copy) or 121 Immunization  Compliance form)
  Cost: $110.00 per child per location (Money Order ONLY)

Monday, June 1, 2020 - Friday, July 17, 2020

Parents must complete an application and provide a copy of the child's 121 
Compliance Form (shot record).
Cost: FREE

Program Begins & Ends: Starts when Jackson Public School begins and ends 

Hours: Monday - Friday, 2:15pm - 5:30pm
(Center closures are followed by the Jackson Public School 's calendar and the City of Jackson, Department of Parks and Recreation discretion).
